Dental Claims
AFLAC
Dental claims are handled on a standardized claim form which is provided to you
by your dentist. This is not a specific AFLAC claim form – it is a
standard American Dental Association (ADA) dental claim form. The same form is used by all dentists
nationwide and by all dental companies nationwide. To file a claim there
are two options:
1. Your dental provider can file the claim electronically for
you. The info they need in order to do this is found on your AFLAC Dental
card or the provider can visit this web site - www.aflac.com/us/en/dentalproviders.aspx
2. The claim can be submitted on paper by mail. The provider
can do this but it is best if you mail it yourself. Using this method takes a little
longer but it keeps you in control of the claim and you will know for
sure that the claim has been mailed. Simply
ask your dentist to print a dental claim form for the procedures you had
performed. They will have the correct form on their computer. The
form will automatically include all of the info AFLAC needs in order to process
your claim. The only thing you will need to add on there is your policy
number. If you do not have your policy
