Flexible Spending Account questions and requests for reimbursement are handled directly through AFLAC Administrative Services. We do not have access to information on your account so all questions must be directed to the toll-free number below.

Please do not fax your reimbursement information for Flexible Spending Accounts to your local agent. This applies to both Unreimbursed Medical and Dependant Care accounts. The link below will provide you with a Request for Reimbursement form and advise you of how to file your claim and where to fax your request to.
